JOB DESCRIPTION SUMMARY

The Peer Support Navigator will be working to provide referrals and peer navigation to individuals who identify as Trans and/or Gender Nonconforming and support them. The ideal candidate would have to be knowledgeable about service delivery in Los Angeles County and surrounding areas. The ideal candidate would be proficient in English and Spanish and will have clear professional guidelines about how to successfully work with people of trans and gender nonconforming experience and be able to make assessments and develop an individualized case plan to empower victims of violence. The case manager would also serve as an advocate and would accompany victims to needed appointments. Case managers must be readily available to work with people of trans experience and support them to develop. Individual service plan and support victims to be empowered and be able to advocate for themselves and other victims of violence from the trans and gender non-conforming community. Ideal candidates must be ready to provide coaching, organize, and conduct sensitivity training about trans issues to organizations that work in the field of sexual assault and domestic violence.

 

ESSENTIAL FUNCTIONS

  1. Conduct comprehensive intake and assessment to deliver effective support services to trans people.

  2. Assist clients with accompaniment to appointments and be able to serve as an advocate to victims of violence who are trans or gender non-conforming individuals.

  3. Actively participate with other organizations funded through this grant

  4. Ensure enrollment of clients are successful and linked to sensitive clinics for other supportive services.

  5. Assist clients with accompaniment to appointments and be able to serve as an advocate to people who are trans or gender non-conforming.

  6. Maintain appropriate paperwork to ensure contract compliance.

  7. Must be able to build relationships with other service and medical providers who are sensitive to providing services to trans/gender nonconforming people.

  8. Organize and coordinate outreach activities along with staff from the Unique Women’s Coalition.

  9. Work in collaboration with other existing staff from The TransLatin@ Coalition.

  10. Generate monthly narrative and quantitative reports to ensure contract obligations.

  11. Provide referrals to other supportive services including but not limited to housing, medical and mental health services, and other services as needed.

 

MINIMUM QUALIFICATIONS - Knowledge, Skills, and Ability

  1. Bachelor of Science Degree in Social Work, Psychology or Public Health related fields or at least 2 years of working with people of trans experience.  

  2. Proof of employment eligibility, valid California Driver’s License and/or California Identification, and yearly automobile insurance.

  3. Updated tuberculosis test (annual requirement) – read, reviewed and dated by a clinician 6 months prior to employment.

  4. Ability to coordinate multiple projects & tasks simultaneously in a high-pressure environment.

  5. Ability to master a wide range of information, taking initiative and adapt in a team environment

  6. Ability to communicate effectively both verbally and in writing.

  7. Ability to work with diverse communities (e.g. multi-gender, ethnic, generation, lingual, Lesbian, gay, bisexual, transgender, substance use).

  8. Basic knowledge of Social Determinants, Workforce Development, HIV/AIDS, and related issues.

  9. Good problem solving and conflict resolution skills.

  10. Great attitude and people skills a must.

  11. Experience and knowledge of PC hardware and software (Windows, PowerPoint, Word, and Excel a must).

  12. Ability to competently conduct trainings and workshops.

  13. Background/Live scan required.

 

NON-ESSENTIAL QUALIFICATIONS - Knowledge, Skills, and Abilities

  1. Relevant community outreach training.

  2. Experience and knowledge of MS Access and Outlook Software.

  3. Bilingual in Spanish and English preferred.

  4. Knowledge of behavior change interventions.

  5. Excellent writing skills.

  6. Four (4) year college degree with emphasis on Public Health related courses or a minimum of 2 years working with trans communities.
