SUMMARY

This Case Manager / Peer Navigator position will work with trans and gender nonconforming individuals who have been released from the CA state prison. This position is under the direct supervision of the Manager of  Reentry & Empowerment  Services. The Case Manager will be working to provide support and direct linkage to supportive services to support trans and gender non-conforming people who have been released from the California Department of Corrections and Rehabilitation (CDCR). The ideal candidate would have to be knowledgeable about service delivery in Los Angeles County and the surrounding areas. The ideal candidate would be proficient in English and Spanish and will have clear professional guidelines about how to successfully work with people of trans and gender nonconforming experience, be able to do intake and assessments, case management, peer navigation services and develop an individualized case plan to empower individuals to be able to reintegrate themselves in to society by providing housing, food and transportation and referral to gender affirming supportive services. The case manager must be readily available to work with people of trans experience and support them to develop an individual service plan. Ideal candidates must have a car, a driver’s license, and insurance. This position will have a six-month performance probationary period upon hiring.

ESSENTIAL FUNCTIONS

  • Attendance and punctuality. 

  • Be available to work at least forty hours per week. 

  • Conduct comprehensive intake and assessment to deliver effective supportive services to trans people who have been released from state prison.

  • Assist clients with accompaniment to appointments and serve as an advocate to people who are trans or gender non-conforming. 

  • Keep track of clients’ appointments and notes, and be clear about keeping track of progress from clients served.

  • Maintain documentation of all client contacts and assist in preparing timely reports subject to review and evaluation of internal and external stakeholders.

  • Must be able to build relationships with partner agencies and provide appropriate referrals to other supportive services.

  • Organize and coordinate outreach activities where trans people congregate and gather.

  • Able to assist a case load of a minimum of 50 individuals. 

  • Support with the supervision of 4 interns a year

  • Support planning and executing a yearly job fair that will include a minimum of 20 potential employers.

  • Other activities are required to conduct the program and achieve the agency's objectives.

  • Must be able to work flexible hours, including evenings and weekends. 

  • Stay in front of the evolving day and agenda. As priorities shift, meetings run long, or unanticipated issues arise, take point on communicating changes to affected persons and facilitating rescheduling, re-prioritizing, or re-assigning to ensure nothing is dropped in transition, and no one is left hanging.

  • Identify and highlight critical timelines and responses.

  • Must have legitimate transportation and current insurance to be able to run occasional errands, appointments and out-of-office site visits as needed.

MINIMUM QUALIFICATIONS - Knowledge, Skills, and Ability

  • Ability to master a wide range of information, taking initiative and adapting in a team environment. 

  • Ability to communicate effectively both verbally and in writing. 

  • Ability to work with diverse communities (e.g., multi-gender, ethnic, generation, lingual, Lesbian, gay, bisexual, transgender. 

  • Handle all administrative tasks related to the Vice President & COO's office, including managing 

  • Ensure that all high-priority messages are highlighted and responded to, in a timely manner.

  • Excellent time management skills, with the ability to prioritize and work with a high level of accuracy within a fast-paced environment.

  • Exceptional attention to detail, highly organized, and proactive in anticipating the department's needs; able to track long-term strategies/projects.

  • Strong communication skills: oral, written, and strong interpersonal communication skills to work effectively with high-level donors and stakeholders.

  • Ensure that all high-priority messages are highlighted and responded to, in a timely manner.

  • Identify and highlight critical timelines and responses.

  • Ability to handle multiple projects and incoming requests from supervisor(s); experience taking the initiative and using independent judgment in non-routine matters.

  • Excellent time management skills, with the ability to prioritize and work with a high level of accuracy within a fast-paced environment.

  • Exceptional attention to detail, highly organized, and proactive in anticipating the department's needs; able to track long-term strategies/projects.

  • Strong communication skills, including oral, written, and interpersonal communication skills, are required to work effectively with high-level donors and stakeholders.

  • An updated annual tuberculosis test – read, reviewed, and dated by a nurse clinician before employment and provided annually to Human Resources.

  • Work weekends or evenings and special events as needed.

NON-ESSENTIAL QUALIFICATIONS - Knowledge, Skills, and Abilities

  • Experience and knowledge of Microsoft Office and Google Workspace.

  • Bilingual in Spanish and English is preferred.

  • Knowledge of specific policy needs and issues of TGI people.

  • Excellent writing and reading skills.

SUPERVISORY RESPONSIBILITIES

  • This position does not have supervisory responsibilities.

PHYSICAL REQUIREMENTS

  • While performing this job, the candidate typically spends time sitting, standing, walking, frequently driving, carrying (max. 20 lbs), lifting (max. 20 lbs), listening, and speaking.

ENVIRONMENTAL CONDITIONS

  • Ergonomically safe office environment with a desktop computer, desk, chair, and natural light from windows.

  • Occasionally in the fieldwork, including but not limited to rallies, community festivals, conferences, nightclubs, and other outreach venues that the target population frequents. Must be comfortable with environmental conditions at locations, such as noise level, aggressive patrons, substance use, etc.  

  • Occasional attendance to community events, conferences, and other meeting venues that the COO frequents. Must be comfortable with environmental conditions at the event locations, such as noise level, aggressive patrons, and substance use and abuse. 

  • As part of our commitment to maintaining a safe and productive work environment, candidates for this position may be required to undergo random drug tests. The drug test may be conducted at any stage of the hiring process and will be in accordance with applicable laws and regulations. 

MENTAL REQUIREMENTS
