SUMMARY

The Community Health Worker (CHW) is a full-time position focusing on community outreach, engagement, and education with The TransLatin@ Coalition (TLC). This position is under the direct supervision of the Community Outreach Program Manager. The ideal candidate will be working to provide outreach services to Trans, Gender Non-Conforming, and Intersex people (TGI) to provide information and link community members to existing resources and services within Los Angeles County, The TransLatin@ Coalition, and other service providers.

The ideal CHW would be proficient in English and Spanish and have clear professional guidelines about successfully working with people of (T.G.I.) experience and their needs. The ideal CHW will be a knowledgeable resource for individuals and families impacted by COVID-19, connecting them to essential health and social services. They will also foster strong partnerships with community organizations to maximize impact.

The ideal CHW must be available to attend community events to promote programs and services and be able to work evenings and weekends. The CHW will be responsible for providing direct outreach services in collaboration with other staff from the TransLatin@ Coalition and the Office of Community Partnerships and Strategic Communications (OCPSC). This position will have a six-month performance probationary period upon hiring.

ESSENTIAL FUNCTIONS

  • Attendance and punctuality.

  • Conduct community outreach, engagement, and education activities connecting clients to essential health and social services, and other TransLatin@ Coalition programs.

  • Collaborate in Public Health in all aspects.

  • Data entry into in-house and internet reporting portals

  • Connect individuals and families impacted by COVID-19 to essential health and social services.

  • Have a good understanding of all the resources to successfully assist individuals/clients with accessing these services.

  • Ensure enrollment of clients into CIVI and other data collecting systems are successful and appropriately entered for tracking and recording purposes to be linked and referred to appropriate programs and services.

  • Assist The TransLatin@ Coalition staff and management team with other supportive services.

  • Generate monthly narrative and quantitative reports to ensure contract obligations.

  • Maintain documentation of all client contacts and assist in preparing timely reports subject to review.

  • Evaluation of internal and external stakeholders.

  • Assist clients with service needs and provide referrals to other supportive services.

  • Actively participate in community events and forums to provide referrals to internal and external services.

  • Ensure that the enrollment of clients is successful in the current services within our organization.

  • Maintain documentation of all client contacts and assist in preparing timely reports subject to review and evaluation of internal and external stakeholders.

  • Ability to speak publicly at various events, including, but not limited to, conferences, health fairs, public forums, meetings, and community gatherings.

  • Organize and coordinate outreach activities where trans people congregate and gather.

  • Work in collaboration with other existing staff from The TransLatin@ Coalition to be able to do presentations to clients about collaboration and recruit from internal existing programs.

  • Other activities as required to conduct program and agency objectives.

  • Must have legitimate transportation and current insurance to run occasional errands, appointments, and out-of-office site visits as needed.

MINIMUM QUALIFICATIONS - Knowledge, Skills, and Ability

  • At least two years of working with people of (TGI) experience.  

  • Proof of employment eligibility.

  • Updated tuberculosis test (annual requirement) – read, reviewed, and dated by a nurse practitioner prior to employment.

  • Ability to coordinate multiple projects & tasks simultaneously in a high-pressure environment.

  • Ability to master a wide range of information, take initiative, and adapt in a team environment.

  • Ability to communicate effectively both verbally and in writing.

  • Ability to work with diverse communities (e.g., multi-gender, ethnic, generation, lingual, lesbian, gay, bisexual, transgender, substance use).

  • Basic knowledge of Social Determinants, Workforce Development, HIV/AIDS, and related issues.

  • Good problem-solving and conflict-resolution skills.

  • Great attitude and people skills are a must.

  • Experience and knowledge of PC hardware and software (Windows, PowerPoint, Word, and Excel a must).

  • Ability to connect with people of trans experience.

  • Other tasks assigned by the supervisor. 

  • Work weekends or evenings and special events as needed.

  • Must have reliable transportation at the time of hire or agree to obtain reliable transportation within the first 12 months of employment.

NON-ESSENTIAL QUALIFICATIONS - Knowledge, Skills, and Abilities

  • Relevant community outreach training.

  • Experience and knowledge of MS Access and Outlook Software.

  • Knowledge using the Google Suite & Microsoft Suite

  • Bilingual in Spanish and English.

  • Knowledge of Transgender, Gender-Expansive & Intersex Communities.

  • Knowledge of behavior change interventions.

  • Excellent writing skills.

SUPERVISORY RESPONSIBILITIES

  • This position does not have supervisory responsibilities.

PHYSICAL REQUIREMENTS

  • While performing this job, the candidate typically spends time sitting, standing, walking, frequently driving, carrying (max. 20 lbs), lifting (max. 20 lbs), listening, and speaking.

ENVIRONMENTAL CONDITIONS

  • Ergonomically safe office environment with desktop computer, desk, chair, and natural light from windows.

  • Outdoor and on-the-field door-to-door interactive canvassing is used at events and in high-traffic areas to target populations.

  • Occasional outreach at community festivals, conferences, nightclubs, and other outreach venues that the target population frequents. Must be comfortable with environmental conditions at the outreach locations, such as noise level, aggressive patrons, and substance use.

  • Occasional attendance to community events, conferences, and other meeting venues. Must be comfortable with environmental conditions at the event locations, such as noise level, aggressive patrons, and substance use and abuse.
