JOB DESCRIPTION

Title: Patient Navigator

Division: CHLA - Gender Affirming Care Clinical Services

FLSA: Non-Exempt, Full Time

Supervisor: Capacity Building & Training Manager

Pay Rate: $26.00 per hour

SUMMARY

This is a full-time position under the direct supervision of the Project Director housed at the Children’s Hospital & The Capacity Building & Training Manager at The TransLatin@ Coalition. The Patient Navigator will be responsible for conducting and facilitating skill-building session intervention with adolescents and young adults, and will participate in chart reviews, case conferences, and relevant training. The ideal candidate will have experience providing case management or access to clinical services to adolescents and expertise in providing services to transgender youth.  This position will have a six-month performance probationary period upon hiring.

ESSENTIAL FUNCTIONS

  • Attendance and punctuality.

  • Collaborates with the team to develop an outreach and program implementation plan.

  • Knowledgeable and skills in youth-specific service delivery systems in the areas of gay, bisexual, and queer health, housing, substance use, and mental health.

  • Bilingual English/Spanish preferred.

  • Excellent communication and organizational skills

  • Conducts outreach to identify potential participants.

  • Supports the development of outreach materials to help youth in project services.

  • Provides education on substance misuse and HIV prevention.

  • Conducts intakes and assessments according to the program plan

  • Provides Navigation Support, including referral and linkage to CHLA’s Gender Affirming Care services

  • Attends internal project meetings.

  • Participates in the SPF process.

  • Supports the development and implementation of social media campaigns.

  • Demonstrates behaviors in concert with hospital mission, values, policies, and procedures.

  • Provide one-on-one sexual health and skill-building sessions.  Sessions include individual assessments, education about risk reduction strategies, and interventions focused on partnering with youth to develop skills to address social determinants of health.

  • Maintain competence and professional development.  Maintain license, certification, or registration as required.

  • Demonstrate behaviors that support performance improvement activities.

  • Provide navigation and case management to at least 70 TGI individuals over the course of the contract.

  • Will be part-time at CHLA and The TransLatin@ Coalition, scheduled days and hours will vary.

  • Participate in at least 40 community/outreach events

  • Other duties as assigned and requested.

  • Must have legitimate transportation and current insurance to run occasional errands, appointments, and out-of-office site visits as needed.

MINIMUM QUALIFICATIONS - Knowledge, Skills, and Ability

  • One year of experience in outreach and/or peer support with gay, bisexual, and queer youth.

  • Two years of case management experience or one year of experience providing health education to youth is required.

  • Two years of experience providing clinical services to adolescents and young adults preferred. 

  • Experience in providing services to transgender youth preferred.

  • Experience in working with diverse populations is preferred.

  • Proof of employment eligibility, valid California Driver’s License and/or California Identification, and yearly automobile insurance.

  • Updated tuberculosis test (annual requirement) – read, reviewed, and dated by a clinician before employment.

  • Ability to coordinate multiple projects & tasks simultaneously in a high-pressure environment.

  • Ability to master a wide range of information, take initiative, and adapt in a team environment.

  • Ability to communicate effectively both verbally and in writing.

  • Ability to work with diverse communities (e.g., multi-gender, ethnic, generation, lingual, Lesbian, gay, bisexual, transgender, substance use).

  • Basic knowledge of Social Determinants, Workforce Development, HIV/AIDS, and related issues.

  • Good problem-solving and conflict-resolution skills.

  • Great attitude and people skills are a must.

  • Experience and knowledge of PC hardware and software (Windows, PowerPoint, Word, and Excel a must).

  • Ability to competently conduct trainings and workshops and coordinate outreach activities.

  • Knowledgeable about trans culture.

  • Ability to respond quickly and in a timely manner to emerging issues as required and requested by any management team members.

  • Have the ability to work weekends or evenings and special events as needed. 

NON-ESSENTIAL QUALIFICATIONS - Knowledge, Skills, and Abilities

  • Relevant community outreach training.

  • Experience and knowledge of MS Access and Outlook Software.

  • Bilingual in Spanish and English.

  • Knowledge of Asian and Pacific Islander Communities.

  • Knowledge of behavior change interventions.

  • Excellent writing skills.

  • A four-year college degree with an emphasis on Public health-related courses or a minimum of two years working with trans communities.

LICENSE, REGISTRATION OR CERTIFICATION REQUIRED

  • Certification as HIV test counselor

PHYSICAL REQUIREMENTS

  • While performing this job, the candidate typically spends time sitting, standing, walking, frequently driving, carrying (max. 20 lbs), lifting (max. 20 lbs), listening, and speaking.

ENVIRONMENTAL CONDITIONS

  • Ergonomically safe office environment with desktop computer, desk, chair, and natural light from windows.

  • Occasional in the fieldwork, including but not limited to rallies, community festivals, conferences, nightclubs, and other outreach venues that the target population frequents. Must be comfortable with environmental conditions at locations, such as noise level, aggressive patrons, substance use, etc. 

  • Occasional attendance to community events, conferences, and other meeting venues that the COO frequents. Must be comfortable with environmental conditions at the event locations, such as noise level, aggressive patrons, and substance use and abuse.

  • As part of our commitment to maintaining a safe and productive work environment, candidates for this position may be required to undergo random drug tests. The drug test may be conducted at any stage of the hiring process and will be under applicable laws and regulations.

MENTAL REQUIREMENTS

  • The individual in this position must be able to accommodate any or all the following: constant distractions, interruptions, uncontrollable changes in priorities/work schedules, availability for on-call/duty after regular working hours, and exposure to inappropriate behavior and language of others.
