SUMMARY
This is a full-time position under the direct supervision of the Manager of Programs for the Violence Prevention Programs. The Peer Support Patient Navigator will be working to Increase access to gender affirming clinical care for 135 TGI individuals ages of 18-25 and provide patient navigation services. The ideal candidate would have to be knowledgeable about service delivery in Los Angeles County and surrounding areas.
The ideal candidate would be proficient, preferably in English and Spanish, and will have clear professional guidelines about how to successfully work with people of trans and gender nonconforming experience and be able to make assessments and develop an individualized case plan to empower victims of violence.
The case manager would also serve as an advocate and would accompany victims to needed appointments. The Peer Support Navigator must be readily available to work with people of trans experience and support them to develop an individual service plan and support victims to be empowered and be able to advocate for themselves and other victims of violence from the trans and gender non-conforming community. Ideal candidates must be ready to provide coaching, organize, and conduct self-advocacy training to TG/NC who have been victims of violence. This position will have a six-month performance probationary period upon hiring.
ESSENTIAL FUNCTIONS
Attendance and punctuality are imperative.
Conduct comprehensive intake and assessment to deliver effective support services to trans people who are victims of violence.
Assist clients with accompaniment to appointments and be able to serve as an advocate for clients seeking gender affirming care clinical services, who are trans or gender non-conforming individuals.
Actively participate with our partners to effectively provide competent services to victims of violence.
Keep track of clients' appointments and notes and be clear about keeping track of the progress of victims of violence.
Maintain documentation of all client contacts and assist in preparing timely reports subject to review and evaluation of internal and external stakeholders.
Must have legitimate transportation and current insurance to be able to transport clients to appointments and training.
Support with outreach activities where trans people congregate and gather.
Able to assist at least 135 individuals in patient navigation and supportive activities.
Other activities as required to conduct program and agency objectives.
Other duties can be assigned by the Program Manager.
Participate in TLC's events, staff meetings, training, and staff development opportunities on an ongoing basis.
Other activities as required to conduct agency objectives related to training and advocacy.
Other duties as assigned and requested.
Must have legitimate transportation and current insurance to be able to run occasional errands, appointments, and out of office site visits as needed.
MINIMUM QUALIFICATIONS - Knowledge, Skills, and Ability
Bachelor of Science Degree in Social Work, Psychology, or Public Health related fields or at least two years of working with people of trans experience.
Proof of employment eligibility, valid California Driver’s License and/or California Identification, and yearly automobile insurance.
Updated tuberculosis test (annual requirement) – read, reviewed, and dated by a clinician prior to employment.
Ability to coordinate multiple projects & tasks simultaneously in a high-pressure environment.
Ability to master a wide range of information, take initiative, and adapt in a team environment.
Ability to communicate effectively both verbally and in writing.
Ability to work with diverse communities (e.g., multi-gender, ethnic, generation, lingual, Lesbian, gay, bisexual, transgender, substance use).
Basic knowledge of Social Determinants, Workforce Development, HIV/AIDS, and related issues.
Good problem-solving and conflict-resolution skills.
Great attitude and people skills are a must.
Experience and knowledge of PC hardware and software (Windows, PowerPoint, Word, and Excel a must).
Ability to competently conduct trainings and workshops and coordinate outreach activities.
Knowledgeable about trans culture.
Ability to respond quickly and in a timely manner to emerging issues as required and requested by any management team members.
Have the ability to work weekends or evenings and special events as needed.
NON-ESSENTIAL QUALIFICATIONS - Knowledge, Skills, and Abilities
Relevant community outreach training.
Experience and knowledge of MS Access and Outlook Software.
Knowledge using the Google Suite & Microsoft Suite
Bilingual in Spanish and English.
Knowledge of Transgender, Gender-Expansive & Intersex Communities.
Knowledge of behavior change interventions.
Excellent writing skills.
Four-year college degree with emphasis on Public Health related courses or a minimum of two years working with trans communities.
SUPERVISORY RESPONSIBILITIES
This position does not have supervisory responsibilities.
PHYSICAL REQUIREMENTS
While performing this job, the candidate typically spends time sitting, standing, walking, frequently driving, carrying (max. 20 lbs), lifting (max. 20 lbs), listening, and speaking.
ENVIRONMENTAL CONDITIONS
Ergonomically safe office environment with desktop computer, desk, chair, and natural light from windows.

Occasional attendance to community events, conferences, and other meeting venues. Must be comfortable with environmental conditions at the event locations, such as noise level, aggressive patrons, and substance use and abuse.
MENTAL REQUIREMENTS
The candidate in this position must accommodate any/all the following: constant distractions, interruptions, uncontrollable changes in priorities/work schedules, availability for on-call/duty after regular working hours, and exposure to inappropriate behavior and language of others.
